Our Doc seen patient for first time for right flank pain and gross hematuria. He documented that on 8/27/12 his BUN was 18 with a creatinine of 1.1. This was a report from another physician but he did review and document. Would that count as 2 data points on and audit?

I would only count it as one. It is just a review of a clinical lab.
