During medical decision making, under the category for Amount and/or Complexity of Data Reviewed, credit can be given for "Review and summarization of old records. What exactly falls into the category of "old records"?

Any takers on this?

Example: If my GI doc sees a patient in follow up and lists colon last year under the past medical history portion and then summarizes the findings, does this count as "old records reviewed/summarized"? What if that colonoscopy was performed a week or two ago?

What is the timeframe to consider a record "old"?

It isn't defined in CPT, my advice would be to check with your local carrier. Try to get something in writing that you can refer to.
