Hi, I’m trying to figure out if a UB04 requires both revenue code and CPT code or it can just be submitted with a CPT code and if it is required on both inpatient and outpatient, please help thank you. Specifically in state of New Jersey Thank you
Thank You Susan.The National Uniform Billing Committee (NUBC) maintains the data specifications for UB04. You can find more about that here: https://www.nubc.org/
Several publishers also produce Uniform Billing Editor Books for specifications on UB04 billing - the AMA and Optum are 2 of them, if you google "uniform billing editor" you can find links to those and a variety of other publishers.
Thank You So Much!You need to include Revenue Codes on UB04 billing. I'm not aware of any payer that will accept UB04 billing without revenue codes.
