Hello,
In need of some insight! We have a newly licensed facility we will be billing for. They are licensed as an outpatient treatment center (physician owned, not connected to a hospital). My assumption is that the place of service should be type "49". Any ideas on what revenue codes should be billed? We initially thought 0490 for the surgical portion, but that indicates "ambulatory", and doesn't seem to be the correct way of reporting this type of facility fee. Is anyone aware of a better revenue code to reflect CPT code examples: 64635, 64483, 64493, 62321 etc? When looking through our options I see revenue code 0511, but still unsure if this will be the correct one to use.
