Anthem has stopped reimbursement of CPT code 77003 with revenue code 320 as of 08/01/2023 stating it's not a covered CPT/Revenue code combination for spinal blocks. I can't find any documentation supporting that change. No changes to that reimbursement with any other insurer at this time. Only Anthem. Billing is outpatient ASC. Does anyone know what the change is and why?
 
77003 is Fluoroscopy guidance and can only be billed per cpt book with these codes:
61050,61055, 62267, 62273, 62280, 62281, 62282, 62284, 64449, 64510, 64517, 64520, 64610, 96450

Not to billed with per cpt book due to is inclusive of the primary procedure are:
62270, 62272, 62320, 62321, 62322, 62323, 62324, 62325, 62326, 62327, 62328, 62329, 64415, 64416, 64417, 64445, 64446, 64447, 64448, 0627T, 0628T

I just searched through my past copies, and it looks like they were making edit updates in 2023 based on NUBC and other sources for valid procedure/revenue code combinations. https://providernews.anthem.com/ohio/articles/outpatient-system-updates-for-2023-1

Rev Code 320 is diagnostic radiology. As much as I get frustrated by nitpicky edit changes, I can understand why that's probably not the best revenue code to report fluoroscopic guidance.

I'm not currently working for a facility, but my best recollection from my last facility position is that we billed it with the surgical revenue code.
