I'm confused about how to code return to work visits. We get patients who have been off work due to illness and they come in for a return to work exam and form to be filled out. Should this be coded as a preventive exam if the illness is resolved and no other problems are found?
 
No, it is not a preventive visit if the purpose of the encounter is to follow up on the illness. I would code it as a problem-oriented visit. If the provider has not yet determined that the problem was resolved prior to the patient's arrival the encounter, then you can still use the diagnosis for the illness, since the follow up visit is still part of the course of treatment for that illness.
