if anyone could please give me any input it would be appreciated...

i have a patient that had a total hip arthroplasty and went to the recovery room. while in the recovery room there was noticed to be a shadow on the x ray . turns out the patient had a periprosthetic fracture and was taken back to the or and a revision was performed.

my question is can i bill the revision with the applicable modifiers since the fracture was present during the first surgery but not seen due to the x ray being shadowed?

You would code the CPT code for the procedure performed when the patient went back to the OR same day along with attached modifier -78 showing that it was unplanned return during the postoperative period to obtain proper payment of the second procedure.

thank you so much i was thinking you could but the dr was saying he didnt want to bill the revison.he just wanted to bill the total. i appreciate your help
