What is best practice when coding Respiratory Failure and neither acute or chronic is specified? Is it safer to code for acute 518.81? I am not able to go back and get confimation from the Dr.
 
Respiratory failure NOS should be coded as 518.81, which is acute respiratory failure. It is correct to use 518.81.

Is it ok to assume respiratory failure when all the Dr. documents are the vent settings under his assessment and plan yet he does NOT document "respiratory failure"??

I don't feel that is an "ok" practice but I'm getting some mixed opinions. Any help would be appreciated.
