I have been auditing physicians' charts for almost a year now. When I complete the audit, I write a report and then meet with the provider to discuss the issues found. I am writing an individual report for every provider which can sometimes seem a little detailed or a little wordy. And frequently takes longer to put the report together than to actually audit the charts. I know I need to keep them somewhat individualized because every provider is a little different.
Does anyone have a structured format they use for reporting audit results? Or any ideas on how to streamline the report. :confused:
