I have a patient who recieved chemo in Jan. for Breast cancer and in Aug. the Dr. states she is in remission not on treatment. Since this is a reoccurring account would I use both the breast cancer code and the history of breast cancer?? if I use both which one should be principal?
 
If there is no evidence of the disease and it no longer requires treatment then it is history of. There is no code for remission of breast cancer. It is either still present or it is not. The only time you would code both history and current is if the the breast cancer is history in one breast and present in the other.
 
I guess that wasn't the best example. What I am trying to ask is ...i'm coding reoccurring accounts, and the beginning of the account they had a cancer being treated with chemotherapy....then towards the end of the year they go into remission. since this is a reoccurring account would I just add the history of cancer along with the present?
