Wiki Rendering Provider for Lab

We have a lab that made the slide and we sent them to a pathologist in a different office to read the slide. We are billing the TC portion but who is my rendering provider that goes in box 33? Is it the name of the lab that made the slide, would it the provider that performed the biopsy, or is the name of the lab director?
