When documentation in the medical record states "Renal Insufficiency, stage 3" Is it appropriate to use 585.3, even though the documentation does not state "Chronic"?

In my opinion, yes it's okay to use 585.3 when worded this way. To me, you couldn't have a stage 3 unless it was chronic.
