denial of 11401
Dr. performed excision of 686.1 and billed out 11401. Humana denied as missing/incomplete/invalid dx code. It was performed on the trunk (clavicle). I've been looking for a better ICD or CPT code to describe the procedure but I am at a loss. Any help? Thank you!
