Wiki removal and reinsertion of spinal instrumentation

Hi everyone!

Please verify whether I am on the right track with spinal instrumentation coding. CPT states "Only the appropriate insertion code should be reported when previously placed spinal instrumentation is being removed or revised during the same session where new instrumentation is inserted at levels including all or part of the previously instrumented segments.

So, if the previous instrumentation was removed from L5-S1, and the new instrumentation was placed at L4-L5, because L5 was included in the old instrumentation and the new instrumentation, I can't charge separately for the removal of the old. Right??

AND...because this instrumentation only spans 1 interspace and has only 4 screws placed in the 4 pedicles per the operative report, I should code 22840 for posterior non-segmental instrumentation. Right?

Thanks for your input! I just want make sure I am understanding this concept correctly.
