Is there a diagnosis or documentation that would allow for more frequent monitoring of patients with heart failure? I understand it is once every calendar month, but some of our patients have severe heart failure and our physicians are wondering if there is a way to get these patients seen more frequently and want to understand the billing issues with this. Thank you!

Why are your physician not using remote CPT codes? This might help but the established patient s ID needs to be verified, minutes added, the mode of using phone or telehealth and must document discussion, list of meds, and chronic conditions.
