Wiki Referring doc needed for E/M & labs

When billing 99213-25, 36415, 80048, etc - does there need to be referring doc in Box 17 on HCFA?
 
Since it's an established visit code, a referring provider isn't needed. I believe that Box 17 is for the referring or ordering physician, so I would put the provider's information that ordered the venipuncture and basic metabolic panel.
