Wiki Records reviewed at length

I am reviewing notes from our Hematology and Oncology and the providers all say that they reviewed the prior records at length. Can I give them any points for this when scoring the amount/complexity of data reviewed. Just want to make sure that I am looking at this correctly.

No, I would not give credit. The ability to assign a data point for that work is only if the records are reviewed and summarized. It's nice that he took some 'lengthy' time to do that, but without a summary of the records and an indication of the records' relevance to the nature of the presenting problem, you really don't have any supporting documentation.
