Re: coding for the eye , each eye for example, with glaucoma, where there are 2 different severities. For example, one eye is at a mild stage and the other moderate, would I have to code the moderate stage 1st?

There are various rules that apply to the sequence of the ICD codes such as etiology, manifestation, sequelae of injuries, multiple codes for a single condition, external causes...etc....it can drive you crazy! :D

In this case if the patient present for eye care and one eye is moderate and one is mild, then yes, you would code the moderate condition first and then the mild condition. You would want to code the severest condition first. Good catch!
