Would like opinions. I have a physician who the dictation either got lost or was not dictated after procedure. The physician when he dictated the note stated in the body of his report: "This patient had arthroscopy menisectomy of this knee on the right and apparently the dictation had not gone through and I am redictating it form the chart, with no memory of the operation.. My question, as I have an uneasy feeling about this, would it be fraudulent if I were to code and bill out? Help
