Do I need to put modifier 25 to the visit when patient has multiple x-rays. The doctors says I do not but when I look up E&M it says to add a 25. I do see denials for inclusive. Please help me with this
 
I have been coding for an ortho practice going on 4 years. I have NEVER added a mod 25 to an E/M ofc visit in conjunction with an xray.
