Wiki Radiofrequency thermal ablation L ramus

Hello fellow ASC coders! I need some help.

We have a pain doc who states that he did the following procedure:

Left SI joint radiofrequency thermal ablation including the L5 ramus under fluoroscopic guidance.

.....The first cannula was placed at the L5-S1 level and the second cannula was placed at the S1 nerve root. After both cannulas were placed ap, oblique, and lateral views were obtained. Confirmation of placement was done. The lesion was induced at 80 seconds, total of four lesions were induced.............

I was thinking of coding 64622-Lt, 64640, 77003-TC

How do you know what is considered paravertebral facet joint nerve and other peripheral nerve from what is dictated above? I think this is where I am confused.
I would appriciated any advice!

Your codes are correct. You will need a mod-51 on the 64640.

The 51 modifier is not an approved ASC modifier - unless the carrier you are billing requires a 51.
