Which date of service should be used to bill the 77334. The date the calculations were done or the date of approval. I have a denial from Medicare. We billed the 77334 on the date of approval and Medicare is saying the date of service is the calculation date on the documentation submitted.

Code 77334 is for treatment devices, code 77300 is for calculations. Regardless the billing date for either is the print date and the approval date should match as this will support and show the required supervision and the physician work as performed at time of service. The billing date of any of the planning services is the print date of the documentation.
