If I code a specific e/m code based on the 1997 guidelines but forgot to look at the 95 guideliness which, could have given it a higher code. Is it considered under coding.?

You have the option to code using either 95/97 and hence it would not be undercoded if you use the one methodology and the other would have a higher code. You need to have a policy which method you use and then stick to it.

thank you

I am soo glad to read your reply because medicare requested notes on our pt but I coded the note based on the 97 guideliness and now that I reviewed the note again it looks like I could have given it a higher code based on the 95 .
