We have always billed Medicare G0434 with a qw modifier and 9 units. Now Medicare is denying 'the number of days or units of service exceeds our acceptable maximum'. No where can I find what the maximum is. I am using the correct dx codes per L32050 effective 3/27/14. Any one have any ideas on this?Thanks!
