Hello All,
We have had several cases of patients who stop coming in for physical therapy without being formally discharged, so our therapists don't know to add the G code for discharge. I've seen guidelines that suggest that a discharge G code isn't required in cases like these, however we are getting edits stating that a third G code is required:
Medicare Outpatient Therapy Functional Reporting Requirements: Other PT/OT Primary functional limitation status HCPCS G8991 cannot be reported alone. A minimum of two appropriately paired functional limitation status G-codes from the same functional group set are required on the same date. One Time Therapy reporting requires all 3 functional limitation G-codes (current, goal, discharge) from the same functional group set on the same date.
Does anyone have any suggestions of how to solve this edit? I do appreciate any help you may be able to give!
