New to psychiatry coding-

Is CPT 99212 the most appropriate E/M code to report for monthly Clozaril monitoring visits by the psychiatrist?

When a psychiatrist sees a patient for a 3, 4 or 6 month follow-up visit for 1 stable chronic condition, is it okay to report 99213 if the HPI-EPF, Exam-D, MDM-SF? Two of the three key components meet 99213, however MDM does not. Our local MAC does not require MDM to be one of the key components.

Any advice would be greatly appreciated.
 
It would depend on the documentation and may vary from patient to patient. A 99212 requires a problem focused history, a problem focused exam and medical decision making of straightforward complexity. I wouldn't be able to tell you which level of service without viewing the documentation. It wouldn't be wise to categorically assign 99212 for a specific sort of visit without viewing the provider's office note first. Not every patient and every visit scenario is the same.
