If the Psychiatrist has done an evaluation/assessment and has a diagnosis, shouldn't that be the primary diagnosis for every visit for that patient?
 
Not necessarily. The initial diagnosis is just that, the initial. Diagnosis can change or the therapist seeing the patient has more information can later determine a different diagnosis. Typically the therapist keeps the diagnosis from the initial eval unless during therapy it becomes apparent there is another diagnosis.
