Wiki Providence denying lines 96415 and 96375

Is anyone else experiencing Providence denying these lines 96415 and 96375? Saying invalid coding and not having a primary code billed.

Here is a scenario. We are billing infusions so we have the J code then used 96413 (which is the primary code) then billed 96415 for the second hour. We billed 96375 with a solmedrol j code for a push.

It is only providence doing this.

Any advice would be appreciated.
