Wiki Provenge and Incident To guidelines?

We recently started Provenge infusion therapy in the office. I went to a seminar and the Presenter stated Incident To doesn't not apply to procedures? Can procedures be billed as Incident To? I've searched on CMS for guidelines regarding this and I haven't been able to find anything.
