I'm looking for guidance on correct diagnosis coding for those being followed for INR levels for a pulmonary embolism and/or DVTs. At what point does the patient transition from it being a "current condition" to a "history of " condition?

You just use the aftercare code for therapeutic drug monitoring with then code for the drug.

Code Also condition requiring care

Under the Z51 section (for aftercare) it says to "code also condition requiring care".
