Wiki proper use of the ABN

I am coding for 2 separate podiatry offices and need clarification on how and when the ABN should be used. One office only has the medicare patient sign a general ABN once a year and thinks it covers every visit for the year. Another office has a patient sign an ABN each and every time they are seen. The ABN's always have the items in box "D" marked that they say may not be covered but the "E" and "F" boxes are never filled out and the pts sign.

Thanks in advance for any insight you may have.
 
A previously furnished ABN is acceptable evidence of notice for current items or services if the previous ABN specified similar or reasonably comparatively items or services for which a denial is expected on the same basis.
An ABN can remain effective for up to one year as long as not other triggering event occurs
