We have a patient who the dr. spend over an hour to go over all her medication & consultation. I don't think it can be justified by using only the 99214. I've did some research & some articles said I can use 99354 on top of 99214 but didn't mention any modifier. Do I need one if I want to bill both?

No 99354 is an add on code and needs no modification all you need is documentation and the times spent documented and you have to back the visit level time out of the total time and have a minimum of 30 minutes left

Could you tell me if there is a modifier that would be used when billing a prolonged service code with a G0101 code to Medicare. The visit was extended and according to MDCR, I can add a modifier to have the 99354 process separately... But this confuses me if it's an add-on code, I don't know why the modifier would be necessary...??? Any help would be appreciated. Thank you.
