Patient had PVP done 5/31/12 then had done again on 10/4/16..insurance has denied stating that PVP is a once in a lifetime benefit..
anyone know anything about this ?

Insurance company is correct. In this case, I believe that your recourse is to bill a 52630, because the doctor was clearly removing residual or regrowth tissue. You may have an appeal on your hands, but this is your best chance for reimbursement

You should use CPT 52630 for the regrowth
