A patient came in for joint pain and GERD, and neither one of those diagnosis support medical necessity for doing the EKG, so if a patient has hypertension, unspecified listed in the problem list of the soap note can that diagnosis code be used with the EKG 93000? :confused:

I am going to say no with the way you have stated this. You need to see in the note why the provider ordered the EKG. Was it known prior to exam that the patient had GERD or was the evaluation trying to determine if it was GERD. You cannot assume the reason for a test, it needs to be clear in the note why a test is being performed.
 
It stated he did the EKG for the patient's shoulder pain (719.41) with impression: EKG wnl, symptoms are reproducible with GH movement.
