Wiki pricing of radiopharmaceuticals aka cardiolite

When billing Cardiolite A9500 with a nuclear stress test, we note the acquisition cost for the charge. Does the amount billed have to match the acquisition cost or can a provider charge a higher amount then the acquisition cost? Do the same rules apply to all insurances?

Cardiolite does fall under radiopharmaceuticals.
