Because pap's aren't required for this age range unless they've been sexually active my provider doesn't complete a pelvic or breast at all. When these girls call they are stating they need apt to refill birthcontrol. So my provider counsels them on safe sex, STI, and proper use and does no exam at all. So obviously I do based on time or low level e/m. We've had multiple call wanting to know why it was charged as preventative because their insurance pays for them at 100%. How does everyone else handle these visits?
 
The way you are coding seems correct. The codes for time (99401-99404) are also labeled "preventative" and would show up like that on an Explanation of Payment. Maybe that is where the confusion lies. Just need to explain how "labels" work in billing.
