Good Afternoon,
I received a denial on the Physical 99385 and the Procedure Paid 10060. A 25modifier would be required, with that being said would I add the 99203 and place the modifier on the Physical as a 25.
He presented for well Man exam , In additional he presented with toe nail injury located Lt big toe and also presented with abscess on left forearm (10060)
The 99203 was not billed.
It does shows two different problems mentioned during the physical that resulted in the Drainage of skin abscess .
I feel like I am missing something.
