Wiki Preprocedure urine culture

Our urologist orders a pre-procedure urine culture prior to some bladder and transurethral procedures. We have been billing these with Z01.812, and they are being denied. Is anyone else seeing this and do you have any suggestions?

This appears to be a possible prophylactic culture and not a culture for treatment of an actual infection. Unfortunately, many carriers will not pay for such a culture for a non specific treatment.
