I was recently told that if your pre op exam is done weeks before the surgery you can bill for the E/M. I have always been under the impression that this exam is part of the global package. Is this really billable?

If the patient is seen for a consult and the decision is made for surgery then the patient comes back for a follow up to the consult ( they refer to it as a pre op exam) then it would be included in the global package regardless of when the exam was performed, a day or week before surgery?

Day prior to or of surgery

The E/M service that is included in the package, per the CPT Surgery Guidelines, is one E/M service provided on the day prior to or the day of surgery. This usually entails an exam and assessment to ensure that the patient is still a viable candidate for the surgery. The pre-op clearances, consultations, and such that you describe in your post can be separately billed.
