Anyone experiencing denials from Medicare (FCSO) "invalid modifier?"
We are a wound care group and bill for debridements sometimes 100+sq-cm.
Ex. 11042,11045, 11045-76,11045-76,51,11045-76,51,59 a total of 100 sq cm. These are usually multiple sites but the total debrided is 100+sq cm. or if anyone coding 11042, 11045 and 4units? PLEASE HELP

Code 11045 is an add on code to 11042, therefore does not need a modifier 51 or a 76.
