When sending out a claim with PQRI measures added to the E/M visit, does the provider need to state somewhere in the note that a given measure was discussed or performed? An example would be in which a dermatology provider is adding PQRI's for colon cancer screening, tobacco use, etc. (things that would not ordinarily be a part of a derm visit). The instruction I have is that we are to leave any and all on the claim and that it will be sorted out during the adjudication process.
