3 bcc a are destructed. Patient comes in for follow up surveillance of skin cancer return. There is no recurrence. Would this count as 3 established or improved on number of diagnoses treated?
 
It's hard to judge MDM without evaluating the actual documentation, but given just this, I would definitely not count it as three problems since the lesions have been removed and there is no recurrence. They are not 'improved' problems - they are resolved, and at this point they are history, not active problems requiring treatment. So the only 'problem' for this visit is the personal history of cancer, which I would count as one point.

Generally speaking, follow-up visits after minor surgeries (if outside the global period), usually only involve straight-forward or low MDM unless there are complications or other factors involved.
