A patient of my podiatrist was admitted to the hospital a month after the podiatrist performed a surgery with a 90 day global period. The patient was not admitted by the podiatrist, but a hospitalist, for reasons that have nothing to do with the patient's previous surgery. My doc was consulted about the ongoing wound care and wound vac for the previous surgery site, but did not do anything at all with the surgical wound, only made recommendations for the patient's stay. Should my doc bill for a consult or would this be considered global? I know the patient was readmitted for a completely different issue so the doc is thinking she can bill with a -24 modifier, but she saw the patient for basic surgical wound care. Any help or suggestions would be appreciated.

I am thinking no -24 modifier. The reason is she saw the pt for wound care for the sx that she did. It she ahd seen the pt for the other foot, or something not pertaining to the sx site, then yes, 24 modifier.
