Our dr does some cyst and lipoma removals in the office. This would be the 21000 codes in the musculature section. Have not had any problems until recently. Some of our carriers say we can't do these in an office setting and today one of our medicaid plans informed us there is POS edit or modifier we should be using but of course she could not tell us. Said it was in the pos edit section of CMS. Any ideas?:confused:
 
POS Problem

Our group has the same issue. This has been a constance battle for the past 2 years. The only success we have had was by contacting our State Medicaid Medical Director and involving our President Surgeon. We have explained our logic to performing these types of procedures in the office setting. They have gone back and adjusted their system to approve certain CPT codes in the musculoskeletal system w/ POS 11 and we have received payment for some claims. This is a very painful and long process! Good luck to you.
