Wiki Pneumo inj w/ a modifier????

Commercial insurance company is denying our 90732 with an E&M visit stating that we need a modifier 25. i've never billed a modifier 25 with this unless we actually charge for the vaccine itself. any advice???

When there is more that a vaccine done we do add the modifier. A lot of payers are requiring this.
