Wiki Please respond ASAP - What modifier to use with J7321

We are billing 20611 and J7321. I billed 20611 (LT modifier) and J7321 and Humana denied for N822 (missing procedure modifier), I then rebilled with LT modifier for J7321 also but again got the same denial N822, so again I rebilled with EJ modifier for J7321 but got the same denial reason. What modifier should I use along with J7321 for Humana. Other insurances do pay without modifier for J7321.

I'm assuming a single-dose vial with no wastage?
