I have any unusual question -- Can religion be part of a chart note? Here is the problem: teenage female wants depo provera shot -- provider only gives that to the married. But in the chart note it states that pt is non practicing Christian. Therefore, provider will not give to teenager.
If anyone has any ideas about this - I would greatly appreciate it.:confused:
 
That would be social history in this case.

Out of curiousity why would the provider only give that to married women?

I had a urologists once that would not do vasectomies because he was catholic, but I don't really think the providers own religous beliefs could count toward anything in an E/M service.

It could be an element of MDM though, in a case where a patient refuses to receive blood products due to beliefs, or other similar situations.

Is that discrimination? Can a MD pick and choose who he takes care of when he is under a contract with insurance companies? HMMMMM!!
