Hi,

I am not sure what CPT code to use for Arthrotomy of Knee with excision of calcified medial collateral ligament.

Post Op dx: Pelligrini-Stieda Syndrome of the right knee.


Procedure: After induction of general anesthesia, the patient's right leg was prepped & draped in usual sterile fashion. An anteromedial incision was made and carried down through the subcutaneous tissue with deep fascia. The capsule was next opened in the line of it's fibers & the superficial medial collateral ligament was incised in the line of its fibers. Upon entering the knee , the superficial portion of the medial collateral ligaments, there was found to be extensive calcification of the fibers of the deep medial collateral ligament. The highly calcified tissue was removed by sharp dissection with a knife and also with rongeur. Intraoperative x-rays were obtained to ensure removal of the calcified ligament. At the end of the procedure, there was found to be about + 1 instability of the medial collateral ligament with 30 degrees of flexion. The wounds were irrigated. The split in the superficial fibers of the medial collateral ligament & the capsule were repaired with 0 Vicryl. the sucutaneous tissue was closed with 3-0 plain. The skin w/ 3-0 monocryl in subcuticular fashion reinforced with Benzoin & Steri-Strips. Sterile dressing & knee immobilizer was applied. The patient tolerated the procedure well. Patient returned to recovery room in satisfactory condition.
