We need help with procedures we do in our Cath Lab. We went to a meeting a the hospital with a radiology specialist and now we are totally confused and hope we are doing thing correct. If someone could please code this for us, we would really appreciate it. We want to see if we have been doing it correct or not.

Procedures Performed:

Common Iliac Angiogram-Bilateral
Superficial Femoral Angiogram-Bilateral
Lower Extremity Angio Bilateral
PTA L SFA
Stent L SFA
Laser Atherectomy L SFA

Indications;
Limiting claudication

Hemodynamics:

BSA (m2): 2.09
Hemoglobin (gm/dl): 14.9


HR(bpm): AIR REST

Aorta: PRESSURE (MMHg)

Peripheral Findings:
Lt Common Iliac (Mid), Calcified 20% lesion
Lt SFA (Proximal), Luminal Irregularities 25% lesion
Lt SFA (Mid), Luminal Irregularities 80% lesion
Lt SFA (Distal), Discrete 70% lesion
Rt SFA (Proximal), Luminal Irregularities 50% lesion
Rt SFA (Mid), Calcified 50% lesion
Rt. SFA (Distal), Discrete 50% lesion

Procedure Notes:
A 6fr crossover sheath was advanced into the left iliac system without difficulty.

Contrast:
Isovue 370 148

Equipment Used:


Intervention Information: A 2mm laser cathether was passed several times at the site of left SFA disease with good debulking results. A 7x120mm stent was then deployed and dilated with a 6mm balloon with excellent flow and results.

Medications Given During Procedure:

Complications:
None

Conclusion:
Successful laser atherectomy and stenting of the left SFA with excellent angiographic results.

Here you go - Initial cath placement was....where? Was this a diagnostic angio or had one been done already? If diagnostic - 75710-26. From where was the Rt SFA injected...at the initial cath placement or did the cath actually move down?

Athrectomy/stent is 37227.

Bilateral Lower Extremity angios, Catheter placed in the Left Common Ilia for Left lower extremity angio, and angio of the Right lower extremity was done through the ipsilateral access site.
Atherectomy/PTA/Stent to Left SFA.

Based on your reply I would code:

37227 for arthrectomy/stent LSFA
75716-26,59 for diagnostic angiogram

The catheter placement is included with the revascularization.
