Pt had fat harvested from periumbilical incision. (20926)
A canthotomy cantholysis was performed with release of the tendon. Underminging was extensive going over to the superficial temporalis fossa lateraly and up above along the eyelid. A small Z-plasty was performed as well. A pocket was then next developed along the superficial tempopralis fossa. The fat graft was then placed along the superficial temporalsi fossa. A pocket was developed along the subcuteaneous muscular aponeurotic system. The fat graft was placed into the pocket and closed with 5-0-PDS.
Attention was next directed towards the reconstruction of the lateral canthal angle. A 5-0 PDS suture was used to reseal the upper and lower tarsal tendons as well as a resuspension of her Whitnall's tubercle along the lateral orbital rim.
Next addressed was the adjacent tissue transfer. Underming was extensive going along the upper lid as well as along the lower lid and superior along the superior oribtal rim. Tissue was then rotated into position with hemostatis maintained. Once this was satisfactory then a combination of 5-0 nylons as well as 6-0 fast absorbing sutures were used to close the incision.
